COVID-19 Precautions

Due to recent California Department of Public Health orders related to the Covid-19 pandemic, visitors to Pioneers Memorial Hospital will be asked to follow the limited and temporary public health requirements as established by the state. Please note that compassionate exceptions can be evaluated on a case-by-case basis for end-of-life, cancer care, special needs, disabled and pediatric patients.

• All visitors to our hospital must show proof of full COVID-19 vaccination*
• Unvaccinated or partially vaccinated visitors will be required to show proof of a negative test report for the coronavirus dated within the last 72 hours at time of visit
• Visitors must be at least 16 years of age or older
• All visitors must wear facial coverings
• All visitors must be screened and be issued a pass
• Hospital visiting hours are 11 a.m. to 5 p.m., daily
• Two designated visitors are allowed per patient during this time.

*Acceptable proof of vaccination status includes vaccination card, photo of card, physician documentation

We understand that these restrictions will be difficult for our patients and their loved ones. We encourage family members to consider other ways to connect with their loved ones via phone, text, social media or video chats. Please contact the department for a video chat appointment with your loved one.

Debido a las órdenes del Departamento de Salud Pública del estado sobre Covid-19, a los visitantes del Pioneers Memorial Hospital se les pedirá que sigan los requisitos limitados y temporales de salud pública establecidos por el estado. Tenga en cuenta que las excepciones compasivas se pueden evaluar caso por caso para pacientes al final de la vida, atención del cáncer, necesidades especiales, discapacitados y pacientes pediátricos.

• Todos los visitantes deben ser examinados y recibir un pase.

As our nation battles Covid-19, California is experiencing an unprecedented and exponential surge in cases. Staffing and other resources have become strained. Pioneers Memorial Healthcare District (PMHD) seeks to continue to provide high-quality, safe patient care to the community. As such, PMHD has developed SARS-CoV-2 Pandemic Crisis Care Guidelines to manage a medical surge event, which places the facility in crisis-care due to a sudden increase in patients and critical shortages of resources. Requesting Medical Records

During the COVID-19 pandemic, we are directing patients to request copies of the records by phone or mail.

Your written Authorization is required before we can fill a request. You can download, print, and complete the form below.

Once you have completed the form, it can be emailed, mailed or faxed to the Medical Records Department via the address or fax number listed below.
